Preparing for your first visit

At High Point Health, we want to ensure your first visit is as smooth and convenient as possible. Here's some essential information to help you prepare:

New Patient Intake Form Mandatory

Parking Considerations:

High Point Health is located in a residential area, and we kindly ask you to be mindful of our neighbors regarding parking. Please park directly in front of and facing the clinic at 5637 30th Ave SW. There are three spots facing the green two-story house. To maintain harmony with our neighbors, please occupy only one parking space. After 4:30 PM, please avoid parking in front of our immediate neighbors' homes. If parking is not available in front of the clinic and you're able to walk, you can park on SW Juneau St., which is just half a block "up the hill" to the south by the pond. If you need assistance, please call us or park in the driveway, and we'll be glad to help. (Parking in the driveway is a last resort.)

Fragrance-Free Environment:

High Point Health is a fragrance-free office. To accommodate clients with chemical sensitivities, we kindly request that you refrain from wearing fragrances, perfumes, or colognes when you come for your appointment.

Nutrition Appointments:

If you're coming for a nutrition appointment, please read and complete the necessary forms before your appointment. Please note that insurance typically does not cover Nutrition Response Testing or Chinese Herbal Medicine.

Acupuncture Appointments:

For acupuncture appointments, if you cannot bring the PDF intake forms already filled out, please arrive 20 minutes early to complete them in our office.

*Please note that fees are subject to change without notice, and package refunds will be prorated at the single treatment fee ($95) for each treatment used.

Acupuncture Insurance Benefits:

We accept various insurance plans, including Aetna, First Choice, Kaiser Permanente, Premera Blue Cross, Regence Blue Shield, United Healthcare, and more. If you have questions about your insurance benefits, please print the High Point Health Insurance intake form and contact your insurance company's customer service for the necessary information. Fill out the insurance intake form and bring it to your first appointment.

For flex plans and other companies, we can provide a "superbill" for you to submit for reimbursement.
At High Point Health, we understand that healing should be accessible. If you encounter financial difficulties or hardships, we work with you to make acupuncture treatments affordable through monthly installment plans, so you can focus on your healing journey.
